Enclosures to be submitted along with requisition letter for issue of examination related documents.

Enclosures to be submitted along with requisition letter for issue of the following examination related documents


(A) FOR NAME CORRECTION

  1. Requisition from the Student through the Principal.
      In the case of the Students who completed their course, can submit directly their   
      requisitions to the Controller of Examinations.
  1. Original Certificate(s) in which correction(s) is/are required.
  2. Photocopy of SSC.
  3. A D.D for the total amount @ Rs. 50/- per Marks Memo / CMM.
  4. A D.D for Rs. 250/- for correction in Provisional Certificate.
  5. If the student name is completely different then Clearance Certificate issued by the University
                  

(B) FOR DUPLICATES

  1. Requisition from the Student through the Principal.
      In the case of the Students who completed their course, can submit directly their
      requisitions to the Controller of Examinations.
  1. A D.D for the total amount @ Rs. 200/- per Marks Memo / CMM.
  2. Duplicate Provisional Certificate cannot be issued as per the University norms.


(C) FOR PROVISIONAL CERTIFICATES & CONSOLIDATED
MARKS MEMOS TO B.TECH. STUDENTS
(against Undertaking in the case of R05 & R07 regulations)

  1. Duly filled in “Undertaking” through the Principal.
  2. Photocopy of SSC.



(D) FOR ADDITION OF GRACE MARKS

  1. Request letter of the student through the Principal.
  2. Original Marks Memo(s) containing the subject(s) for which addition of Grace Marks, is applicable.
  3. Undertaking for issue of Provisional Certificate & Consolidated Marks Memo. when students get 216 or 220 credits after adding grace marks.
 (E) FOR CLASS CHANGE
[Change from > 49.85 % to 50 % for Pass Class to Second Class
Change from > 59.85 % to 60 % for Second Class to First Class
Change from > 69.85 % to 70 % for First Class to First Class with distinction]

  1. Requisition from the Student through the Principal.
      In the case of the Students who completed their course, can submit directly their
      requisitions to the Controller of Examinations.
  1. Originals of (i) Provisional Certificate, (ii) Consolidated Marks Memo &           (iii) Final Semester marks memo.


(F) FOR PROVISIONAL CERTIFICATES & CONSOLIDATED MARKS MEMOS 
(if issue is pending for want of Photograph / Father’s name)

  1. Requisition from the Student through the Principal.
      In the case of the Students who completed their course, can submit directly their   
      requisitions to the Controller of Examinations.
  1. Photograph of the student attested by the Principal along with a spare photograph or Photocopy of SSC or both.


(G)  PERMISSION FOR SCRIBE TO APPEAR FOR EXAMINATIONS

  1. Medical Certificate issued by a Civil Surgeon working in a Government Hospital.
  2. Photo of the student / candidate highlighting the inability to appear for the examination.
  3. The Particulars of proposed scribe i.e., name, address, qualifications and present occupation. [The scribe should be of intermediate qualification with arts subjects only].
  4. A letter from the scribe stating that he /she is willing to act as scribe.
  5. A copy of the certificate of scribe’s qualification along with recent photograph duly attested by the Principal.
  6. A letter from the Principal stating that he/she personally verified and satisfied regarding qualification of the scribe as per norms and that he/she will provide a separate room and invigilator for all the examinations of the candidate.

Note: All Demand Drafts must be on any Nationalized Bank drawn in favour of “THE REGISTRAR, JNTUH” payable at HYDERABAD.
